Health History Form 

The Maryland Institute College of Art (MICA) requires completed health history forms. MICA Health and Wellness will be sending you these forms, but for your convenience they are listed below.

The Health History Form includes areas for your health history, a physical examination, and your immunization history. Up-to-date immunizations are required for all new students. Please complete the areas indicated and have your primary care provider complete the physical examination and immunization history sections. Please bring these forms filled out and signed to student orientation. More information regarding the MICA Health and Wellness Center over Summer Session can be found here

Shopping Shuttle RSVP 

Would you like to have access to a shopping shuttle on Saturday June 15th?
 
It will leave from the Meyerhoff house at 10:30AM. 140 W Lafayette Ave, Baltimore, MD 21217. The shuttle will go to the shopping center, Canton Crossing which includes a Harris Teeter (grocery store), Target, Five Below, Michaels, and many more shops and restaurants! We need 10 students signed up in order to run the shuttle, so please let us know if you're coming below! We need to know by Friday May 29th.

Welcome to Baltimore! Orientation Reminder!

 

We are excited to meet our NEW Summer students for Orientation on Friday, June 14th to share the many resources and support services at MICA! We'll see you at 9am in room 160 of the Lazarus Center.

Students can park in the "Studio Center" East or West Lots, on each side of the Lazarus Center -  click here for an interactive parking map. Wear comfy shoes (you will be taking a tour of the campus) and plan to bring a water bottle.
